Nampa Christian is blessed to have two amazing nurses serving our students. They base out of the elementary building and keep tabs on medical authorizations, medications, and other medical-related items that keep our students operating at their physical best!

If your student has a medical condition or allergy, please read through the forms below and fill out the ones that apply. These forms can be turned in to the front desk of any NCS building.
Authorization to Administer Medication
Please complete this form if your student ever needs medication to be given at school (i.e. short-term antibiotic, daily prescription medication, emergency medication such as an inhaler or EpiPen).
Contract for Self-Carried Medication
Please complete this form if your student carries their medication with them (i.e. insulin, inhaler or EpiPen).
Diabetes Medical Management Plan
Please complete this if your student has type-one or type-two diabetes.
Asthma Action Plan
Please complete this if your student has asthma.
Anaphylactic Allergy Plan
Please complete this if your student has an anaphylactic allergy and uses an EpiPen (i.e. anaphylactic allergy to food, bees, etc.).
Medical Condition Action Plan
Please complete this if your student has a medical condition not covered by the specific plans offered. May also be used for non-anaphylactic allergies.
