Please complete these steps to accomplish the application process. Links to online or PDF forms are included.

FIRST

Fill out the Application for Admission ($50.00 fee due in interview process)

SECOND

Bring these items to the interview:

  1. Most recent report card

  2. Most recent Achievement Test results (ISAT, Iowa Basic Skills, Stanford, etc.)

  3. Birth Certificate
    A certified copy from the state in which the student was born – hospital certificate is inadmissible. The school will make a duplicate copy. In compliance with IDAHO STATE CODE (Sec. 18-4511) requires positive identification of any new student enrollee. The student and/or parents must provide the school with such identification within thirty (30) days of enrollment. The accepted proof of identity is a CERTIFIED copy of the student’s birth certificate.
    The following “reliable” proof of the student’s identity may be used only if a certified copy of a birth certificate cannot be provided and it must be accompanied by an affidavit explaining why the birth certificate is not provided: Passport, Visa, or other government documentation of student’s identity.

  4. Immunization Record
    Idaho State law requires that all immunization records be “up to date” and on file prior to a student attending school.

    Children enrolled in Preschool (4 years & older) – 2nd grade must have:
    • 5 doses of DTaP (4 doses meet requirements if at least one dose was administered after the 4th birthday)
    • 3 Polio
    • 2 MMR
    • 3 Hepatitis B

    Children enrolled in 3rd – 12th grade must have:
    • 4 doses of DTaP
    • 3 Polio
    • 2 MMR
    • 3 Hepatitis B (for those students born after November 22, 1991)

  5. Completed Statement of Faith form - download and print

  6. Completed Parental Commitment form - download and print

  7. Completed Student Commitment (Grades 7 – 12) form - download and print

  8. Completed Personal Recommendation form - download and print

  9. Completed Church Recommendation form - download and print

  10. Completed Elementary Dress Code (Grades 1 – 6) form - download and print

  11. Completed Secondary Dress Code (Grades 7 – 12) form - download and print

    All of the above items represent critical information needed by the School Registrar in order to process your application and schedule your interview. Your assistance in completing these items is greatly appreciated.
